We value your time and understand that you have a busy schedule. We have convenient and expanded office hours that allow you to reserve an appointment that is prompt and convenient for you and that fits easily into your active day.

We pride ourselves on the efficacy of our office.  We always strive to stay on schedule and we are proud of our near perfect  ‘zero’ patient waiting time.

Your first visit is all about you. – Your comfort, your convenience, your happiness, your dental health.

When you come to your first appointment, bring your smile but also remember to bring these things in with you as well:

• Your Insurance card and Medicare Card
• A list of all the medications you are taking [you can obtain an up-to-date printed list from your pharmacist at no cost]
• Your completed Medical History Forms
• Your completed Smile Evaluation form [optional]

For your convenience, you may download the Medical History Forms and Smile Evaluation Form and fill them out in advance in the comfort of your home or at our office. Our team will be more than happy to assist you.
